This form gives consent from Year 12 IB students to QTAC to share their IB Admission Score (IBAS), with the school they completed their Year 12 studies in. This form is for Queensland IB School students to complete on their own behalf, to provide consent to obtain and share their IB Admissions Score. The IB Admissions Score is only allocated and used within Australia for students completing their IB Diploma in Year 12 in Australia. IB students completing their IB offshore do not need to complete this form. Consent for receipt of IB Admissions Score
By completing this form, you are giving your consent to QTAC to supply you with your IB Admissions Score. Additionally, if you consent, your IB Admissions Score will be supplied to the school you completed Year 12 in for their records.
Regardless of whether you are consenting to receive an IB Admissions Score or not, if you are a Queensland Year 12 IB student (beginning in 2023) you will have an IB Admissions Score calculated and used for tertiary admissions purposes within QTAC.
You only need to complete this form once. Please ensure you receive the confirmation email upon completion. If you do not receive it, and it has not gone to a spam or junk folder, please complete the form again using a different email address. QTAC will only supply you with your IB Admissions Score if you provide a personal (not a school) email address in the form below.

Unless you (a current or recently graduated IB student) provide consent here, QTAC will not send an email to share with you, or your school of attendance in Year 12, your IB Admissions Score.
Only with your consent will QTAC disclose individual IB Admissions Score as calculated by QTAC, to you via the personal (not school) email you provide in this consent form.
QTAC will supply you and if you also consent, the school you attended in Year 12 with your IB Admissions Score, via email, once calculated.
