TTT Applicant Declaration

    I acknowledge and agree to the following:

• that it is my responsibility to provide all necessary information and documentary evidence in support of my application.  I understand that my application may not be processed without the provision of this information;
• that QTAC may cancel my application and institutions may withdraw any place offered and cancel my enrolment at any stage before or during a course if I provide incorrect, incomplete and/or misleading information or if it can be shown that I was ineligible or not sufficiently competitive to gain entry to a course;
• that if I am a current Year 12 student, QTAC will supply my preference, offer and response data to my school for the purpose of assistance and counselling;
• that QTAC is authorised to collect, verify, retrieve, store, and transfer information with respect to my application and if necessary provide information to educational institutions, admissions centres and relevant government bodies.  I understand that QTAC is not responsible if any government body/educational body/institution does not supply these records;
• that QTAC act on my behalf and transfer my details to the Department of Industry, Innovation, Science, Research and Tertiary Education (DIISRTE) and that DIISRTE will collect and store my personal information for use in connection with the Higher Education Information Management System;
• that the information I provide to QTAC may be used in the compilation of QTAC reports and where appropriate, be provided to any government department, statutory authority, institution or other relevant organisation that QTAC deems suitable.  I understand that QTAC is not responsible should these bodies use this information for purposes beyond which QTAC disclosed;
• that I accept the consequences (ie forfeit any right to equal consideration) if by any action, failure, or omission I prevent QTAC from processing and completing its processing and assessment of the application before the scheduled offer rounds.  This includes not meeting the relevant deadlines and/or not paying the appropriate fees in accordance with instructions relating to the QTAC application process;
• that QTAC can contact me regarding my application using print or electronic communication methods;
• that if I am unsuccessful in my preferred courses I authorise QTAC to inform me of other course options; and
• that I have read the application instructions and accept the conditions of application.
