Documentation Guidelines

The following information details whether you must supply supporting documentation to support the qualifications accepted for assessment by QTAC.  If your supporting documentation does not follow these guidelines the qualification cannot be assessed.

When documenting a qualification:

  • never send irreplaceable original documents such as your Senior or Degree certificates
  • any documents you submit become the property of QTAC and will not be returned
  • documents lodged with QTAC in previous years have not been retained and need to be resubmitted
  • in all cases QTAC reserves the right to sight an original document

Secondary Studies

You are not required to provide documentation for the following studies and years:

  • Queensland Senior Certificate since 1975

  • ACT Year 12 Certificate since 1977

  • NSW Higher School Certificate since 1969

  • SA Year 12 Certificate or SA Certificate of Education since 1966

  • TAS Higher School Certificate or TAS Certificate of Education since 1983

  • VIC Higher School Certificate or Victorian Certificate of Education since 1975

  • WA Tertiary Entrance Examination since 1976

  • International Baccalaureate results since 2002 (provided that the B2 form has been submitted to the International Baccalaureate Organisation (IBO) nominating QTAC as a recipient of your results)

  • NZ National Certificate of Education (NCEA) results since 2004 (you should provide your National Student Number (NSN)

You must provide supporting documentation for secondary studies undertaken anywhere else. You must provide a certified copy of your certificate of results showing all subjects undertaken, results achieved, and proof of award of your high school certificate/Year 12 certification.

If QTAC is unable to locate/verify your results from the information you provide, you will be contacted for further details.  

Bridging and Preparatory Studies

Please refer to QTAC's Institution List (requires Acrobat Reader) to determine if you are required to provide documentation. 

If you are required to provide supporting documentation for your studies, you must provide a certified copy of your academic record showing all subjects undertaken, results (including failures) and a key to the grading system.

Tertiary Studies

Please refer to QTAC's Institution List (requires Acrobat Reader) to determine if you are required to provide documentation.  

If you are required to provide supporting documentation for your studies, you will need to provide a certified copy of your academic record showing all subjects attempted to date, results (including failures), a key to the grading system, name of the award and degree certificate if applicable. This is available from your tertiary institution. Memoranda of results, statements of attainment and transcripts down-loaded from the Internet are not acceptable.

If you were ever excluded, refused further enrolment or placed on academic probation, you should attach a statement giving reasons why you believe you can now study successfully.  Some institutions may take this information into account when assessing your application.

If QTAC is unable to locate/verify your results from the information you provide, you will be contacted for further details.  If you have any outstanding fees payable to your tertiary institution, QTAC will be unable to access your results and charges may apply.

Certificate, Advanced Certificate or Other Studies

Please refer to QTAC's Institution List (requires Acrobat Reader) to determine if you are required to provide documentation.  

If you are required to provide supporting documentation for your studies, you will need to provide a certified copy of your academic record showing all subjects attempted to date, results (including failures) and a key to the grading system.

Trades and Traineeships

All trades and traineeships must be documented. You will need to provide a certified copy of your certificate showing trade or traineeship completion from the relevant government or issuing body, such as Training and Employment Recognition Council. 

Other Postsecondary and Professional Qualifications

All postsecondary and professional qualifications must be documented. 

Depending on your qualifications, appropriate documents might include certified copies of the following:

  • Armed Forces statement of service

  • AMEB (7th Grade or higher) award certificate

  • National Accreditation Authority for Translators & Interpreters award

  • Commercial Pilot's Licence (CASA)

  • Nursing Registration (licences are not acceptable)

  • Police Force statement of service

Employment

All employment experience must be documented. You should provide a statement of service or signed reference that is dated, on official company letterhead and outlines the position(s) you held, duties performed and length and dates of service (specifying full-time or part-time).

Admissions Tests 

Special Tertiary Admissions Test (STAT)

You do not have to provide any documentation for the Special Tertiary Admissions Test (STAT):

  • if you sat the test through QTAC in any year
  • if you sat the test through any other interstate admissions centre or agency since 1996.

You must provide documentation for STAT sittings administered by an agency other than QTAC prior to 1996. You would need to provide a certified copy of your official statement of test results. 

Undergraduate Medical Admissions Test (UMAT)

You do not have to provide any documentation for the Undergraduate Medical Admissions Test (UMAT). You should however enter your Candidate number if available.

Special Consideration of Educational Disadvantage

Requests for Special Consideration must be documented. You must complete the Application for Special Consideration of Educational Disadvantage (SCD) form and attach supporting statements from an appropriate authority with knowledge of your case, such as doctors, psychologists, ministers of religion, solicitors, school principal, and university lecturers.  

People with Disabilities

Please complete the confidential questionnaire that will be provided for you.

Personal Competencies Assessment

Requests for Personal Competencies Assessment must be documented.  You should provide a concise statement (no more than five A4-sized pages, word processed, typed or neatly handwritten) addressing the six PCA Categories together with supporting documents and statements from referees.

Foreign Qualifications

Any qualification undertaken in a foreign language must be documented by a certified copy of the original transcript and accompanied by an official translation in English.

English Language Proficiency

If your first language is not English and you have not previously undertaken at least one year of secondary or tertiary study where the medium of instruction was English, you are required to demonstrate your English Language Proficiency. 

To demonstrate English Language proficiency, you can undertake a English Language proficiency examination. You would then provide a certified copy of your official statement of test results.
